We will provide account assistance so you can continue to use your device and stay connected. If you are a survivor of domestic violence, please contact Clarity Wireless Customer Service at 1-877-870-8901 (or dial 611 from your Clarity Wireless) or visit a Clarity Wireless store location.

If you are not located in New York State, additional documentation will be required to process your request. Please provide any one of the documents listed below after you contact Clarity Wireless:

  • Signed affidavit from licensed medical or mental healthcare provider; victim services provider OR employee of court acting within the scope of employment.
  • Police report or Police statements provided to Magistrate/judges.
  • Charging documents.
  • Court Order.
  • Protective or restraining orders.
  • Any other official record that documents the domestic violence.
